Nurse-driven virtual-provider support for peak surge times

To support the on-site physician at the NW FEDs during surge times to improve efficiency of care, decrease door-doc times and prevent LWBS’s.

Should be driven by nursing, but if the physician is drowning and needs help, then they should ask the charge nurse to implement the “TeleProvider Surge Support”.

NOTE: Can be used for any level patient - the TeleProvider will see the patient, enter orders if needed, and can even discharge patients.

Objective

Process

  1. During times when there is a large influx of patients or when the nurse or tech appreciates a need for virtual-provider support, then notify the Charge Nurse, who will first contact Nichole Campbell, NP at 817-228-6490 via call or text the message “Teleprovider surge support requested” from the Charge Nurse iPhone. (Her contact is the only pinned contact at the top of the text messages) See video below.

    1. If Nichole is unable to do the virtual visits, then she will text the “TeleProvider Surge Group” and that provider will call the Charge Nurse iPhone to notify them.

      1. If no response within 10 minutes, then the charge nurse calls Dr. Poage’s cellphone from the Charge Nurse iPhone.

  2. Tech then unlocks the TeleProvider Surge Support iPad (the PIN is on the back of the iPad) See video below.

    1. Tech selects “APP Surge Support” icon on the iPad —> then “Northwest Emergency at Town Square” —> then select the provider that responded to the text.

    2. Tech enters “tsq teledoc” in the “patient name” field —> OK to allow camera and mic access.

    3. Once confirmed connection to provider, tell the provider the patient’s name, and then face iPad to patient to establish visibility.

    4. After encounter, leave iPad logged in, and move iPad to the next patient and tell the provider the next patient’s name.

    5. Once all visits are complete, tech will close out safari browser and turn off iPad.
